This is the final call for participants for the June 26-27th Gazelle/Cypress meeting at NIST. I need to have all visitors registered by Friday. See the information below.

 

For those who have already indicated to me that you’re attending, I will send out additional logistics in a separate e-mail in the next few days.

NIST is hosting a face-to-face Gazelle/Cypress design meeting on June 26^th and 27^th. Steve Moore has sent out the details of the agenda.

 

All visitors to NIST are required to pre-register; I need the following information:

 

For U.S. citizens:

1)            First Name, Middle, and Last Name

 

IMPORTANT: You must provide a valid government issued ID (e.g., a driver’s license) to gain admission into NIST.

 

For Non-U.S. citizens:

1)      First Name, Middle, and Last Name
2)      Date of Birth (Month, Day and Year)
3)      Place of Birth (city, Country )
4)      Passport Number 
5)      Issuing Country
6)      Visitor Title/Position
7)      Gender
8)      Employer/Sponsor
9)      Telephone
10)     Address
11)     Citizenship
12)     Countries of Dual Citizenship (if applicable)
13)     Country of Residence
14)     U.S. Permanent Resident : YES  or  NO
15)     Date (s) of Visit:

IMPORTANT: You must have your passport with you to gain admission into NIST.
