Connectathon demographics: missing "City" field in the Gazelle CSV export

Hello Eric,

After exporting the test patients from Gazelle ("Connectathon ->
Patient Generation and Sharing"), we noticed that the field "City" is
missing in the CSV file.
We really need the CSV file in order to upload the patients into our
PIX_Manager before the connectathon.

We would appreciate it if you can look into this issue.
