MediaWiki Internal Error on Create Account (User::addToDatabase: hit a key conflict attempting to insert user 'SomeUserName')

This not a problem with the iDempiere system, but with the wiki instead. 

When I try to create a new user in order to login, I an error message about a key conflict attempting to insert user.

Note that I have not created the user before, so it is not a duplicate.

For example, if I enrol a made-up user name "SomeUserName:, I get the same error:

Hi Gerald,

This error has been hitting us intermittently since some time.

I just noticed you're trying to create the user coming from the english wiki (w-en)

Can you please try with the main wiki and give us feedback if that solves the problem?

Sorry for the slow response.

Yes, I can confirm that the problem does NOT occur on the main wiki page. I am now successfully enrolled on the system!

Many thanks for your assistance and good luck with hunting down the cause. Intermittent errors are the worst. At least you have one extra data point :-)
