This page here implies that if we had to sign a release of claims in order to receive severance, we can "double dip" and receive UI benefits at the same time.  They require a copy of the severance agreement, so be sure to keep a copy of that.

Max initial payment is $674/wk.

Hi Deb -- I used 1 Rogers St 02142 and the phone number used here for the main operator, (617) 577-8500.  

There is a question about where I physically worked and how many people work there and were laid off there.  I work at home, and I was the only person laid off in my house, so I was a bit unclear what to put.  I effectively pretended I worked full time in Cambridge, and answered "Don't know" about number of layoffs there.

Thats where it went for me as well. 

Interestingly, I don't get notified on my phone for emails that go into that folder... only for emails that end up in my standard inbox. I haven't logged into gmail in a few weeks and have missed a number of emails due to that...

At one point in the application it mentioned that I needed to send in the separation agreement as proof that I signed a claims release for IBM.   But I don't see where/how I am supposed to send this.   Does anyone know?   Did you have to send this?

After you fill out everything and submit the application, go to your UI home page, and then look at the inbox or messages. There will be something indicating an incomplete form, i.e. the missing severance agreement. Follow the clicks and it will have you print out a page with a barcode to mail in along with the severance agreement.

Let me know if that doesn't get you there.

We did get severance and vacation pay, I am not sure why it is asking for since Sunday July1st 2012 ??

That looks right to me, that's what I put.  I think they want to know if we received any other severance in the last year that would potentially disqualify us from receiving unemployment.
