strange error during regridding

Hi al

I am doing 3D flow in a curved tube with an inner solid, as shown in attached figure. I got good case with coarser grid in z (nz =20), but when I increase nz to 30 or 36/40, I got below error during regridding

The iBAMR is the recent master branch.

Anyone knows the reason

At beginning of timestep # 56
Simulation time is 0.056
