A supplementary registration is a special type of registration that may be used to correct an error in an existing registration or to amplify the information given in an existing registration. For detailed instructions on completing the online application, see the Compendium of U.S. Copyright Office Practices, Chapter 1800, Section 1802.8.

Specifically, the number should consist of a twelve-character string, without spaces or hyphens. For published registration numbers with a prefix of TX, VA, PA, or SR enter the two letter prefix followed by ten digits. For unpublished registrations with a prefix of TXu, VAu, PAu, or SRu enter the three letter prefix followed by nine digits. Every letter of the alphabetic prefix should be capitalized, and zeroes should be inserted between the prefix and the numerical portion of the registration number so that the total number of characters is twelve.

When you go to the polls to vote, present one of the seven (7) acceptable forms of photo ID. If you do not possess an acceptable form of photo identification and cannot reasonably obtain one, you may present a supporting form of ID (one of which is your valid voter registration certificate) and execute a Reasonable Impediment Declaration (PDF), noting your reasonable impediment to obtaining an acceptable form of photo identification, stating that the information contained in the declaration is true, that you are the same individual personally appearing at the polling place to sign the declaration, and that you face a reasonable impediment to procuring an acceptable form of photo identification. An election official will match your name to the official list of registered voters.

For voters that applied for and received the permanent exemption from showing a photo ID based in their disability status, this status will be notated on their voter registration certificate. These voters are required to present their voter registration certificate when they go to the polls to vote.

The Copyright Office is changing the format of certain copyright registration certificates, as part of a pilot project. Unlike traditional certificates of registration, the certificates issued as part of the pilot project are not facsimiles of the paper applications, but are electronically generated from data entered from the applications. Since the new certificate format was first announced, the Office has made some further modifications. All certificates issued as part of the pilot project are in the format described below. (Traditional format certificates will continue to be issued for works registered outside the pilot project.)

The Office is reengineering its business processes to improve the efficiency and timeliness of its public services. To test the new processes, a pilot project began on February 14, 2005, involving registrations for motion pictures and other audiovisual works registered in class PA. During the pilot, applicants continue to file claims using the current Form PA paper application, but the certificate is system-generated. While the substantive content of the new certificate is almost identical to that of the current one, the format and general appearance are significantly different. For further background information, see

Since the new certificate format was first announced, the Office has made some further modifications. As modified, the pilot certificate differs from the traditional certificate in the following ways.

Registration is required for Government UA. All Aircraft (UA) owned by agencies, offices or subdivisions of: the United States (other than aircraft (UA) of the U.S. Armed Forces), the States, the District of Columbia, or a territory or possession of the United States must be registered in accordance with 14 C.F.R. Part 47 or Part 48.

An N-number to be assigned to the registered UA.

If a special N-number was reserved in advance by the UA owner for this registration, this number will be assigned if it is entered on the forms in the indicated blanks.

A special N-number may be requested when filing the application and other documents for registration. Include a letter that lists several N-number choices, and the $10.00 special number fee. The first listed number verified as available will be assigned to the aircraft (UA).

A random N-number will be assigned at no cost if the indicated blanks on the registration forms are left empty, or a random number is requested.

Registration Fees:

Pay the $5.00 registration fee by check or money order made payable to the Federal Aviation Administration. Multiple fees may be consolidated into a single payment. Registration and N-number fees are waived when the applicant is a Federal, State or local government office, agency or institution.

Where to Register:

Send your registration documents to the FAA, Aircraft Registration Branch. Addresses for regular mail and overnight courier deliveries are available through the Contact the Aircraft Registration Branch menu item.

Homemade boats require an affidavit from the applicant as to the origin of the boat and proof of the materials used in the construction of the boat (i.e., invoices from marine and hardware dealers for parts, etc.). Homemade boats are required to be inspected and issued an hull identification number (HIN) by a TPWD game warden.

It is illegal to possess or intentionally sell, offer for sale, or purchase a vessel or an outboard motor with a hull identification number or serial number that has been altered, defaced, mutilated, or removed. To acquire registration or title for a vessel or outboard motor with a serial number that has been altered, defaced, mutilated or removed, a sworn statement must be filed with TPWD describing the vessel or outboard motor, providing proof of legal ownership, and stating the reason for the destruction, removal, alteration or defacement of the number. This statement must be accompanied by a fee of $25.00 and an inspection certificate from a game warden stating the vessel or outboard motor has been inspected. This section does not apply to USCG Documented vessels.

Any boat with a permanently installed marine sanitation device (MSD) is required to obtain and MSD certification and display a certification decal if the boat is operated on any waters in Texas, including coastal waters up to three nautical miles. For certification and decal information contact the Texas Commission on Environmental Quality (TCEQ) or call (512) 239-0400.

If you are a registered vendor but have not received the Business Registration Certificate, you may obtain a certificate online. Please note that this certificate is not required for all businesses in New Jersey. It is required only for those doing business with the public sector and with the casino service industry.

You may check the online registration inquiry to determine if the business is already registered. If you have not registered but are required to have this certificate, you will need to complete Form NJ-REG. Representatives of the Division's Customer Contact Center activity are available to assist in the registration process. Call 609.292.9292.

You must register if you are a person who offers for transportation or transports in commerce a shipment containing any of the following categories of hazardous materials (including hazardous wastes):

Since the beginning of the program in 1992, activities in Categories A through E above have required registration by law. Category F was added by regulation beginning with the 2000-2001 registration year.

Clarifications: The HMR apply to persons who offer for transportation or transport by highway, rail, water, or air. Water transportation does not include hazardous materials loaded or carried on board a vessel without benefit of containers or labels and received and handled by the vessel carrier without mark or count. This type of shipment, for which the vessel is the container, such as LNG or oil tanker vessels, is under the authority of the U.S. Coast Guard in the Department of Homeland Security.

The term "person" means each separate legal entity, such as a corporation, partnership, association, or LLC or LP. This means that a separately incorporated subsidiary (or LLC or LP) must register if it engages in the specified activities, even when a parent company is also required to register. All locations and vehicles operated by a person are covered by that person's registration.

Requirements for Hazardous Waste Generators: All hazardous wastes subject to the Hazardous Waste Manifest Requirements of the U.S. Environmental Protection Agency (EPA) are hazardous materials. A hazardous waste generator must register if you meet the requirements identified above.

After you click on the "Search" button, a list of possible companies will be displayed. The most recent registration number for each company is given next to the company name. Click on the "Details" button at the end of the line to see a short report of the company information with each of the registration numbers associated with that company record. The most recent registration is given at the bottom of the list. Click "View/Print" to see and print a copy of the certificate.
