Subject: Your Filled Out Form 6 and next steps

 

Dear Sandeep Sadashiv Jangam,

Congratulations on taking the first step towards becoming a registered voter.

You now need to:

  1. Print the application form here
    1. If above link doesn't work for you then copy-paste this link in your browser:
      ----
  2. Sign the form on page 2 after Section IV Declaration.
  3. Submit the form along with the self-attested documents mentioned below to your central collection point on campus or in your office. Alternatively, you can submit it directly to your local election commission's office in your constituency.
    1. Your campus coordinator will collect the forms and work with the Corporate India Voting team to facilitate the submission to the respective election commission office.
    2. Check to see if there are any camps scheduled for your campus (ask your leadership to initiate), or see if patriotic and civic-minded volunteers from your own corporate colleagues can assist with the submission. Alternately, you can click here to get a list of the nearest coalition partner IVBI representative or call 088-8888-8888 and ask for the Volunteer For a Better India representative in your area and see if they can help.
    3. Please email indiavo...@gmail.com in case there is no representative available near you and we will work to help you know of your nearest election office.
  4. If you decide to submit the application forms in Electoral Registration Officer (ERO) directly, they will give you an acknowledgment slip for your application form.
  5. Documents required for registration (self-attested documents)
    1. One recent colored passport-size photograph along with the form. The photograph will be used to print your image in the electoral roll and issue of identity card.
    2. Name and Age with documentary proof (even though the explanation section of Form 6 states that birth proof is not required if you are over 22, our experience has shown that certain offices still insist for it, so we are suggesting that you include it)
      1. Indian Passport OR
      2. PAN Card OR
      3. Driving License OR
      4. Birth Certificate issued by Municipal Authorities OR
      5. Aadhar letter issued by UIDAI OR
      6. Marksheet of Class 5 or 8 or 10 if it contains date of birth OR
      7. More examples are listed in the explanation section that is printed along with your Form 6
    3. Proof of ordinary residence (duly certified copy to be attached) (refers to place where the person has been staying ordinarily not necessarily permanently i.e., where you live right now even if you don't live there permanently (as long as you have been there 6 months)
      1. Bank / Kisan/ Post Office current pass book OR
      2. Ration card, driving license, IT Assessment order, IT Return, Passport OR
      3. Latest rent agreement; OR
      4. Latest water electricity/telephone/gas bills in name of applicant or immediate family member, spouse or parent OR
      5. Any post/ letter/mail delivered through Indian Postal department in the applicants name at the address of ordinary residence.
      6. More examples are listed in the explanation section that is printed along with your Form6
    4. Special situations for students OR renters.
      1. If a student staying in a hostel, a "Student Declaration Form" which states the stay in the hostel signed by the Principal/Dean/Registrar
      2. If staying on rent, a NOC letter from the owner and the rent agreement
    5. Additional documentation – If possible a copy of the Voter ID card of anyone in the person's family or anyone close to the person's residential vicinity (this will dramatically decrease the risk of getting rejected!)

Frequently Asked Question regarding Section IV Declaration: the declaration section, as it appears printed on Form 6 may appear to be little confusing (it is a statutory form that can't be changed). The first two points are rather straightforward.

  1. In our opinion, points 3 is only intended to ensure that you have not already applied in this election cycle for an election card at this or any other address, because if you already recently applied in this election cycle you shouldn't be registering today - you should be following up on that application.
  2. Point 4 asks you to declare either
    1. you have never had an EPIC card before, OR
    2. if you have had one, or if you think you may have had one at an old address, you must provide all known and easily available information about your old card so that it can get cancelled.

Note, if you don't remember the EPIC number, you can try to find it on line, but if that doesn't work, please at least fill out the address where you lived so the ECI can try and find the EPIC number and cancel it. You are not supposed to have multiple cards and so this section will help to get the old one cancelled while allowing you to apply for the card at the current address.
