Until we arrange to have events posted on our website where short alerts can then be sent to you to see “what’s new”, some new information will arrive in a cluster like this. Two separate events for your consideration are contained in the attachments and a new job opportunity is addressed below.
CARE TRANSITION COACH
Peninsula Agency on Aging (PAA) is currently seeking applicants for the position of Care Transition Coach to assist individuals age 65+ to avoid preventable hospital readmissions. Occasional weekend and evening coverage required. Frequent local and occasional overnight travel.
Successful candidates will possess excellent oral, written and interviewing skills; ability to relate to elders and family caregivers from diverse cultural backgrounds; ability to work well with individuals who have literacy and/or physical limitations; basic knowledge of health care industry; computer literacy; excellent organizational skills; ability to work independently as well as collaborate with a team; knowledge of Person Centered principals and services available to older adults helpful; valid driver’s license required.
Degree from an accredited college or university in human services or health care field and at least 2 years of related experience, preferably with the senior population.
Full and part time positions available. Drug free workplace, AA/EOE
