Physician Documentation

Hello

Can someone explain to me why there can be physician documentation within 24 hours after severe sepsis presentation date and time if certain criteria used for severe sepsis presentation was due to something else and we should not use it  but then  we have to have physician documentation within 6 hours of severe sepsis presentation date and time  if patient did not have severe sepsis/septic shock.
