Viewing mammograms in Horos

Hello,

We'd be grateful for some advice on using Horos to view mammograms as we are setting up a digital teaching museum in our breast unit.  We have two main problems:

1.  In contrast to CT for example, where you can specify a certain window width/level for display of soft tissues or bones etc, mammograms are slightly more complicated in that mammographic density varies considerably between individuals and it is therefore not possible to have a one-size-fits-all window setting.  As such we are finding that we are having to rewindow images each time we open them, partly because the default mammo window setting isn't appropriate for 90% of mammograms, and partly because Horos doesn't seem to retain the window settings from the previous time the image was opened.  We'd really appreciate some suggestions to get around this problem.

2.  We would really like to have a mammography 'hanging protocol' feature which mimics the way mammograms are displayed on a standard PACS viewer.  For example, you would usually view the two MLO views back to back, and then be able to toggle between this and the two CC views displayed back to back.  Similarly, it would be great to be able to view previous comparison films in a standard way i.e. MLOs back to back with current on the inside and previous on the outside.  Would there be any interest in incorporating this into the next version of Horos or writing a plug in for it, as breast imagers would find it extremely useful.

Hello Megan:

First, thanks for using and supporting Horos!  Regarding your issues,

1) We'll take a look at the retention of window settings globally to see if that is in fact a bug.  If it is, we will document it on your behalf in the Issue Tracker and make the developer community are of the issue.

2) Supporting hanging protocols for mammography is certainly on our radar.  If you'd like to formally request this as a feature, please document it the Issue Tracker.  That said, Horos is not approved for diagnostic interpretation of breast imaging by the ACR, and therefore should only be used in research or review of imaging studies.

Hey. Just wondering if you ever got a satisfactory to your question. I have a question as well. I'm trying to verify that the GSPS data is actually being burned to disc from my primary PACS and when opening in Horos I can't find any way to display CAD marks, annotations, etc. Is there a way? Thanks

Dear Megan and or Phillip Jackson, I am using Horos to display cases for teaching to my registrars (in these COVID times a lot of teaching is now remote and online). The mammograms hanging is a problem, as detailed by Megan. Is there any solution yet to either question: the default windowing and a reasonable mammogram hanging protocol - with images paired and centre justified, zoomed to match the window size and matching the contralateral breast images - Say back to back CCs, for example? Is there a plug-in perhaps?
