A potential cause for this is related to iCloud Drive synchronization implemented by macOS since Sierra.
Loss of data can happen in case synchronization of Desktop / Documents folder is active on two or more stations. Horos database is kept in Document folders, but if you have two mac computers, both with Horos installed, same Apple ID, and synchronization of the referred folders active, data corruption/loss can happen.
In version 3.0 of Horos, which is planned to have release candidates in early February we will be handling/warning about this situation right in the software.
In attachment, screenshots that shows you how to disable the synchronization I mentioned.
Just be careful if disabling this function won’t affect your work in other directions.
Sorry for this inconvenience, we are already working to make Horos compatible with this Apple feature.

Thanks for the quick reply:) I don’t use iCloud drive but I double-checked to make sure that folder wasn’t selected. My database is also just on one computer. When I first imported this particular study, I used the option to link the files rather than copy to the Horos database... so my source dicoms were all in a folder outside Horos.
I just re-imported the same study, and chose to copy the files directly to the Horos database this time, and everything appears to be working fine. It seems like the links to the source files might have been broken somehow...not sure why. The only thing I’ve done since the last time I was in Horos was a Time Machine backup.
