Respected Sir,

I would like to inform you that as per instruction received from State HMIS Cell O/o Mission Director (NRHM), Haryana to rectify the data of DHIS-2 from April 2013 to December 2013. I repeatedly mail the issue in DHIS-2 to all facilities for correction of data. I have mailed near about 25 mails but i did not get any reply/feedback from CHC Jhojhu Kalan, I called a meeting of all IAs for rectification of data. But i received a mail from CHC jhojhu Kalan through DPM Bhiwani that due to urgent work IA can not attend the meeting. Every time i call IA Jhojhu Kalan i received a same kind of response that due to urgent work IA cannot attend the meeting. Instead of taking a ownership of data everybody in CHC Jhojhu Kalan is ignoring this work. Moreover IA whose prime responsibility is of DHIS-2 & HMIS, he also not taking his work seriously. Instead of repeated & repeated reminders he never respond back and due to this the DHIS-2(Child Health) Data of CHC Jhojhu Kalan is nothing more than a garbage. Please look at the attached reports the Child Health data of CHC Jhojhu Kalan is completely empty in some of the month no Birth Dose given by the Staff Nurse & in September 2013 staff nurse had immunized from BCG to Measles and even VitaminA doses. I am planning to finalizing the data but only because CHC Jhojhu Kalan i find myself in a doubtful situation in doing so. I strongly recommend to terminate the services of Sh. Sunil Kumar, IA Jhojhu Kalan for such a irresponsible behavior.

I am attaching the previous mail received from CHC Jhojhu Kalan for exemption from meetings for IA.

I am also attaching the prof of errors in DHIS-2 Data.

Pls take cognisance and issue a show cause explanation. If found unsatisfactory the action as deemed fit has to be taken. 

This kind of insubordination will not be tolerated.
