Respected mam,

It is requested to issue direction to concern districts officer/official for data collection and  providing data in HMIS format for district HQ at data entry point of Civil Surgeon  office  ( DMEOs ) on monthly basis regularly . Necessary instructions are being issued  to all DMEOs for ensuring data capturing and its validation with concern programme in-charge at district.
