Just in case anyone else will be upgrading to Star 21, I have some information for you. During our allergy integration testing we found an issue where cancelled allergies (from HCI) were still showing up in Star and thus Physician’s Portal also had the same incorrect information.
I opened a case with the upgrade team, and it seems there is a known bug in the upgrade and it will require a fix at the MSE level. Below is the comment form Star support

I have found the issue – it is a bug in the MSE program responsible for processing date/time stamps.
I have contacted MSE technical to have it corrected and will let you know when that has been done.
