Written policies on parental notifcation

We are formalizing a policy on notifying parents/emergency contacts in various situations, and I'm looking for examples. I know this was discussed recently, and I have read through all of those very helpful threads. I found the policy from University of Wisconsin - Eau Claire, but I'm looking for others that any might be willing to share.
