Does anyone know how to change a diagnosis to primary using VGR? We are recreating the list which Scott / Dr. Pratt demo'ed at the Insight Conference to discontinue a diagnosis. I have been able to populate and discontinue, but our docs requested the ability to make one of the diagnosis on the list a primary from the iForm.

I thought there wasn't a way to actually populate the diagnosis from VGR (and therefore iForms)!  How do you do it?  The following command line in VGR did brings up the diagnosis order, but doesn't populate it with the correct diagnosis and instead prompts the user for a search.

EDIT,SET,,orderstring,CAT,"@LOAD_ORDER=OIS=12006154^^^OOS=12000158^^^12000056=Acute systolic heart failure"

Any insights?

Peter, you have to be on ER12 to populate Diagnosis from an iForm.

Note that the LOAD_ORDER syntax will be slightly different (it requires a different prompt in which you enter the SNOMED, if I remember correctly) since the diagnosis is supposed to be codified, but I'll have to get back to you with more specifics.

Ok, I've got the syntax in front of me now and I just remembered to come back and update this thread.  Your example would look something like:

EDIT,SET,,orderstring,CAT,"@LOAD_ORDER=OIS=12006154^^^OOS=12000158^^^12000191=Medical^^^SNOMED_CODE=443254009"

You need to designate your OIS and OOS and usual, tell the system that it's a codified medical diagnosis, and provide the SNOMED code.  If you don't know the code, you can find them by going into verbose mode, then doing a diagnosis search in HEO; the SNOMED codes will show up inline.

Piggybacking on this thread as it seems appropriate...

I could use some assistance with this. I am trying to populate Diagnosis with an iForm, and have used the suggested syntax.(EDIT,SET,,orderstring,CAT,"@LOAD_ORDER=OIS=12006154^^^OOS=12000158^^^12000191=Medical^^^SNOMED_CODE=385093006")

We are on ER 12.

Yet, when I order the Diagnosis, it only shows the code on the ADCVAANDISML screen (like so: ! 385093006). It does not show the description, nor does it appear to be properly codified, as the text is in black instead of purple. In fact, I can change the value of the SNOMED prompt to whatever I want (e.g., "SNOMED_CODE=oreo cookies" and that will show up in the left pane.

If I place HEO in verbose mode and order the diagnosis manually, it shows the SNOMED code by the description... so I know that the code exists for that diagnosis and is correct.

Any ideas? Surely there is some fundamental thing I am doing wrong and the answer is right in front of my face.

Try changing your string to "@LOAD_ORDER=OIS=12006154^^^OOS=12000158^^^12000191=Medical^^^SNOMED_CODE=385093006^^^SNOMED=Community acquired pneumonia"

I think you may just be missing the "SNOMED" prompt.

Have you confirmed that 385093006 is a valid SNOMED code?

Mark, thanks for the suggestion. I added the "SNOMED" prompt and the behavior is exactly the same.

Scott, yes I am sure that 385093006 is the code for CAP. If I search for CAP in HEO debug mode, the SNOMED code is displayed next to it.

One more thing of note: I'm not sure if this is typical behavior, as I have only started working with Diagnoses today, but when I order a diagnosis, be it through an iForm or manually in HEO, it seems to order twice for some reason (when in verbose mode - it shows the confirmation "OK to process this order?" twice).

Anyone else have this behavior? Could it be related to my issue?

It may be a long shot, but do you have codified diagnosis turned on (site label), or are you allowing free text diagnoses?

Thanks for the replies, guys.

Dr. Pratt with McKesson helped me with this issue. As it turned out, one of the prompts on the orderable was not configured as "mandatory". He changed the value and now it works just fine.
