Blue screen of death when safetly remove usb hardware

I have Windows Professional V2002, SP3 and keep getting a fatal error
(blue screen of death) when I use the "safetly remove hardware" icon
in the bottom right tray.
I have a mini dump file but can't figure out how to attach it. Can
someone help?
