Hello Everyone,

Liability Waver

Everyone needs to fill out the new updated online form. The new form replaces the paper waiver forms. The submission will save to a spreadsheet which can easily be checked.

If you are going to be in the lab please complete the new form. The QR code will also take you to the new form.
