どちらかというと、ヘルスケアペイヤーや医薬品メーカーなどが主導的な役割を果たしてきたDisease Managementに対し、プライマリーケ
アの現場が主導的な役割を担うPatient-centered Medical Homeへの注目が集まっています。クラウドコンピューティングの利
活用が期待される領域ですが、セキュリティ、人材育成など課題も山積しています。日本での適用は可能でしょうか。

「Medical Home」は、元々、1967年に米国小児科学会（American Academy of Pediatrics）が提唱した概念
であり、子どものカルテの集中管理型アーカーイブに関わるものでしたが、2002年以降拡張されて現在に至っています。
米国小児科学会では、「Medical Home」を、「a model of delivering primary care that is
accessible, continuous, comprehensive, family-centered, coordinated,
compassionate, and culturally effective care」と定義しています。

なお、米国では、“Seniors’ Medication Copayment Reduction Act of 2009”という法案が、民主党
Stabenow議員および共和党Hutchison議員により上院に提出されました。Medical Homeは、Medicare改革の柱の一つに
位置付けられていることから、診療報酬制度上のインセンティブに関する取扱いなどで関わってくる可能性があります。
