Physician Referrals to Health Coaches

Physicians in the Independence Blue Cross Network are now receiving
more data about their patients through IBC's Smart Registry
and have the ability to refer patients into a health coaching program
from this data.

Previously the reports included patient information about key clinical
tests and treatments that are prescribed for patients with certain
chronic conditions, such as diabetes, heart failure, coronary artery
disease and asthma. Now the reports indicate whether or not the test
was completed and shows results where available. Physicians can then
take this data into account for referral into IBC's disease management
program, Connections, which includes a telephonic health coaching
component.

What referal strategies is your organization using to enroll
participants in health coaching programs?
