hData Record Format - Minor Changes

I have made some minor changes to the HRF document. My concern is our namespace handling, especially for the root document schema. These changes should be considered administrative - in fact, once HL7 decides to move the documents into DSTU, we will need to revisit the namespaces anyways.

I would appreciate any comments.
