(Standard Form 85P)
available on the web

And it seems to be explained here:

When filling out the 85P, it obviously isnt a security clearance
rather there are different levels of 'public trust positions' and
associated risk levels that are assigned by the federal government.
The type of investigation that is conducted depends on the 'level of
trust' and level of 'risk' of the specific job. Most agencies use
these levels: Low, moderate, high.
A low risk position may have the following investigation type:
Fingerprint check, NCIC check.
A moderate position may have:
National agency check (NAC) which is a national police check, FBI and
other agencies.
A NACI (National Agency Check with Written inquiries). Basically they
send letters of inquiry to specfic persons or places of your
particular background going back 5 years.
High Risk may have this investigation:
Could include what is called an LBI (limited background investigation)
which involves a personal inverview with an investigator, a national
agency check, credit check and written inquiries to specific areas of
your background going back AT LEAST 5 years. Personal interviews may
be conducted in specific areas of your background going back at least
3 years. Depending on the agency, more in depth investigations may be
utilized for high risk positions. Each agency has their own standards,
but are generally the same across the board.
