I listened to this and found that the VA doesn't know what VistA costs to run annually. USD 1.0 billion is one estimate, but that includes all the third party commercial software associated with the various medical equipment that supplies data to VistA as well as IT hardware costs, licencing fees (InterSystems Cache licencing cost USD 37.7 million back in 2008) and cloud hosting costs. Since many of these things won't change when moving to Oracle Cerner, or any other solution for that matter, others suggest that supporting VistA (which has no licence fees) costs more like USD 100 million annually, most of which, I'm guessing, is payroll, both internal and external.

Meanwhile the estimates for implementing Oracle Cerner also vary wildly, but do show a distinct trend for increasing dramatically year on year:

2018 VA USD 10 billion

2019 VA USD 16 billion

2022 IODA USD 49.8 billion

February 2025 GAO USD 100+ billion (this may have been a verbal comment made during a congressional hearing with no concrete evidence to back it up)

September 2025 VA USD 37 billion
