You might find the VA's VistA AI Analytics project of interest.
The linked VAA report discusses how to capture and preserve all this
provenance and structure in TIU notes (which is over 85% of the content
of TIU notes!) that is otherwise lost - all with no modification to
VistA or CPRS.
