Condition Status Date

I'm on R5 https://hl7.org/fhir/R5/condition-definitions.html and trying to figure out how to record a date when a condition changes. Example is Patient is giving medical history. The patient is diagnosed with cancer. The patient then goes into remission one year later, the patient then has a recurrence, cancer then goes into remission etc. I want to be able to record when the status changes and the date. How do I do this? The onset date description in theory can be used to capture the status date change by creating multiple conditions for the same diagnosis. Fine with doing that but i would rather keep the onset date the same even if I create multiple conditions and just change the status and associate a date with the status. Any help is appreciated.
