Trying to acquire a station ID without success

I posted this to the HamSCI PSWS group but thought I would post it here as well.  I need a little assistance. 

I'm at the point in the WSPRDaemon build that I need to add a new station. I've acquired all of the components for the RX888 WSPRDaemon SDR PSWS. 

Following the instructions from https://github.com/HamSCI/PSWS_Documentation/wiki/HF-wsprdaemon-Receiver,  I've tried to acquire a station ID via https://pswsnetwork.caps.ua.edu/stations/add/ without success. 

I've tried several browsers with JS enabled. After login and completion of the 'Add New Station' , it just returns to the form again without generating a PSWS_STATION_ID 

Hopefully, I am attempting this right. Any help would be appreciated
