Shifting of Registered Office One State to Another State

I would really appreciate it if anyone would kindly share a complete set of ROC filing documentations (including RD-1 - Petition format and Form INC-26 new paper publication format) pertaining to shifting of the registered office of a private limited company from one state to another.
