Form FC-1 Query

Hi Folks,

We recently submitted Form FC-1 for registration of a Liaison Office in India. 

The form has been marked for resubmission with the following remark:

"Attach list of directors in proper valid format with all details as prescribed and required to be duly notarized in terms of Rule 9 of the Companies (Registration of Foreign Companies) Rules, 2014."

Upon reviewing Rule 9, we could not find any prescribed format for the List of Directors. The Rule specifies the particulars to be provided but does not appear to prescribe a specific format.

Could anyone please confirm whether there is any standard or accepted format being followed for this purpose? 

Any guidance or sample format would be greatly appreciated.
