Form ADT 1

Is it mandatory to fill in Form ADT 1 the previous SRN number of the Statutory Auditors being now appointed again after cooling period of 5 years

The Firm of Statutory Auditors can be appointed for maximum 2 Terms of 5 years each and can be appointed again after cooling period of 5 years.

 Whereas Individual CA can be appointed maximum for 1 term of 5 years and again after cooling period of 5 years. This requirement has been followed.

The question is whether SRN number of previous appointment  need be mentioned in Form ADT 1 even after cooling period of 5 years.
