Issue with Lead Generation Component

This component seems to be out of date.

Once installed all the fields are displayed with a NULL value for the placeholder.

IS there a replacement out there....

Can you please try using the component from this link instead? In Google Web Designer, select + button in the Components panel and browse to the zip.
