Attached is a message from Enoch Choi translated by Kwan Woo Kim. I am guessing that this is how Enoch wants the Midterm files. 

I gather from his message that you need to submit your midterm to me by Thursday (on the group Dropbox) with a signed printout of the attached Excel document.

Print out the attached Excel file and sign, then with your e-file of test submit it to your department head by Thursday.

If you would not have mid-term test, please write your justification on an A4 paper and submit to your department head.

Your department head will check the following:

1. Any misspelling or

2. wrong choice of words.

3. Does the test have an adequate number of question for 75 minutes?

 

To minimize the unexpected mistakes on the mid-term tests is the purpose of doing this, I plead.

중간고사 출제문제에 대해 점검하신 후 싸인 하실 수 있는 서류를 완성하였습니다.

 

선생님들께서는 첨부되어 있는 엑셀파일을 출력하신후, '교과'란에 자신의 싸인을 하신 후, 학과장 선생님들께 중간고사 문제와 함께 목요일까지 제출하시면 될 것 같습니다.

 

혹시 지필고사로 중간고사를 보지 않으시는 선생님들께서는 A4 종이에 간략히 지필고사 대신 어떤 평가를 하시는지에 대해 적어주시고, 학과장선생님께 제출해주시면 될 것 같습니다.

 

학과장 선생님들께서는 체크하실때, 1) 오타가 있는가? 2) 문제상의 용어가 적절한가? 3) 시험문제 수등을 고려하여, 시험시간(75분)에 적절한가? 등을 점검해 주시면 좋을 것 같습니다.

 

중간고사를 미리 점검하는 이유는 시험지 출제를 미리 점검하고, 학생들이 시험을 보기전에 실수를 최소화하자는 이유임을 이해해 주시고, 도와주시면 감사하겠습니다.

So this is not an approval form found online but something that we have to print out, sign, and ... scan and attach with an email? Do we need to get you to sign the paper form? Is it only the department head that we need to sign, or should we find the other person too?
