Using Nextiva VOIP and Clio (Boutique subscription)

My VOIP is Nextiva and I am using Clio at the "Boutique" subscription level

I've downloaded the gUnify add on and it shows up in Chrome

When I click it and go to the gUnify home, it wants me to sign-in/register.

I tried to register but I can't get it to allow me to choose a Broadsoft Service Provider (no options appear)

Also, I called Nextiva and they did not have any information on obtaining a Licensing Key.
