PR #278: Add primary and foreign ID to field types, reclassify existing fields

PR 278 addresses issue #266, clarifying the various roles ID types play in the spec, and adding a section to each table reference that clarifies which field or fields are required to uniquely identify a row.

Voting is open on PR 278.

The vote is for adding a primary key attribute to each table's description and labeling "unique" and "foreign" IDs where applicable.

Voting ends on 2021-10-14T23:59:59Z.
