Ok so I downloaded gtalk and its asking me for my username and password but this is the 1st time using this and I don't have a password. What do I need to do to set this up??

You can use all google services with the same account. Just create a gmail account and enter the username and password in gtalk.

I have a problem with logging into google chat on my phone after installing this app. Is this a known issue?

Some people has got this problem. It's generally solved by itself after 1 or 2 days.
The best option is to use a separated account for GTalkSMS for your phone.

The way i have my phone set up is i have 2 google talk / gmail accounts.
One for gtalksms and my regular account.

Create both accouts, use google talk or another xmpp client and add the accounts to each others buddy list (may not be required)

Install gtalk sms on your phone, enter your username/password for your phone's gmail account.
In the address to notify add your regular account.

You should then be able to logon to gtalk or another client to control your phone.
Gmail also includes a google talk client so you should be able to use this to test.

When sending sms to a person by name, its asking me to choose between two numbers that are the same. Is there a way to get it to always use a number or have it check for duplicate phone numbers and resolve on its own?

Check if you have a standard phone number for the contact set.
Also, if you get a duplicate number, there must be a cause for this, so take a look if the numbers are entered twice for the contact.

You were connected in Wifi or Data (GSM) because I think that we
cannot be connected in GSM voice and data in the same time.
