The Mac App Store sees the GrowlHelperApp inside Growl.prefPane 1.2.2 (or older) as a previous installation of Growl, and “helpfully” “upgrades” it by installing Growl.app inside the prefpane.
The result is that deleting the 1.2.x prefpane also deletes the 1.3 app that the MAS put inside.
(This wasn't caught in beta testing because betas are distributed directly, not through the MAS. As far as I know, there's no way to distribute betas through the MAS.)

First, make sure Growl isn't running. It shouldn't be, but this is nonetheless the first thing to check. Check this in Activity Monitor and kill it if it is.
Next, try quitting and relaunching the App Store app.
If that doesn't work, try holding down option when you click on the Purchased icon. That shows *all* purchases, regardless of whatever exclusion criteria it normally uses.

Certainly doable. Try to move Growl.app out of the prefpane(s) first, then delete the prefpane(s). The uninstaller does its thing with root powers anyway, so this shouldn't be too difficult.

There is no way to test Mac App Store distribution without distributing through the Mac App Store. They did as much testing as they possibly could.

One thing to try is option-clicking the Purchases icon. That definitely used to work, but didn't for me.
Another to try is to simply search your machine for Growl. Any Growl.app, GrowlHelperApp.app, or Growl.prefPane, move it to the Trash, and empty the Trash (deleting them all for real) when you're done. Even if they're not in the normal places which would be truly installed, MAS will consider it installed if it finds it *anywhere*.
Settings files don't matter. You can keep those.
If you found any leftover Growls, then, after you've deleted them all, go back to MAS—maybe quit and relaunch it just to be sure—and look in Purchases again.
