แจ้งการจ่ายยา Ticagrelor 90 mg ในผู้ป่วย Pre PCI

ด้วยผู้แทนยา Ticagrelor ได้นำยาตัวอย่างมาให้ Cath Lab ใช้ แพทย์จึงสั่งให้ยา Ticagrelor เพื่อเตรียมผู้ป่วยก่อนสวนหัวใจ

ดังนั้นหากเป็นคำสั่ง Ticagrelor 90 mg 2 tab PO Stat "ห้องยาไม่ต้องจ่ายยา"

ให้หอผู้ป่วยใช้ยาตัวอย่างจาก Cath Lab เอง และถึงแม้ Cath Lab มาเบิกยาคืนโดยเขียนมาในสำเนาคำสั่งแพทย์ ก็ไม่ต้องจ่าย

หากแพทย์ หรือ หอผู้ป่วย ต่องการยาจากห้องจ่ายยา ให้ผู้ป่วยชำระเงินค่ายา Ticagrelor ก่อน จึงจ่ายยาได้

เพราะระเบียบ PTC ให้ใช้ "หลังสวนหัวใจ เท่านั้น" เพื่อป้องกันการเป็นซ้ำ

- บัตรสุขภาพ ประกันสังคม เฉพาะผู้ป่วยที่เกิด Stent thrombosis แม้ได้ Cloplidrogel 75 mg

- เบิกได้ จ่ายตรง ชำระเงิน ใช้แทน Cloplidrogel หลังสวนหัวใจ ในผู้ป่วยที่มีความเสี่ยงสูงต่อการเกิด Stent thrombosis (แพทย์หัวใจพิจารณาเอง)

นอกข้อบ่งชี้ที่ PTC กำหนด ให้แจ้งผู้ป่วยชำระเงินก่อนจ่ายยา

ยาตัวอย่างในที่นี้หมายถึงอะไรคะ เป็นยาแถมพิเศษ หรือยาใหม่ที่ยังไม่มีในบัญชี หรือ อื่นๆ 

เนื่องด้วยตามนโยบายของ PTC ไม่ให้มีการวางยาตัวอย่าง

กรณีตามที่กล่าวมา ไม่ทราบว่าแจ้ง PTC ทราบแล้วหรือไม่อย่างไร ซึ่งควรที่จะนำเสนอพิจารณาในประเด็น indication ที่ใช้เป็น pre-hospitalization เป็น FDA label หรือไม่อย่างไร แล้วจะส่งผลกระทบอย่างอื่นหรือไม่ อย่างไร

อันนี้เป็นข้อมูล หรือ สิ่งที่เกิดขึ้น มีการนำยาตัวอย่างมาให้หมอลองใช้ ส่วนจะเข้ามาได้ยังงัย หรือ มีการแจ้ง PTC หรือ ไม่ ก็ไม่แน่ใจว่าเป็นหน้าที่ใครที่ต้องจัดการ แต่เป็นปัญหาในการทำงาน ซึ่งต้องแจ้งผู้ปฏิบัติทราบ และเข้าใจ ในสิ่งที่ควรทำและไม่ควรทำ ครับ
