remote authentication denied

We currently have a user who is encountering issues while trying to authenticate using the widget. The error they receive is: "Remote authentication denied from xx.xx.xx.xx This user is not authorized to access this service remotely. If you believe this is an error, please contact your domain administrator." where xx.xx.xx.xx is an ip address of one of our servers.

Other users can authenticate correctly. Has anyone seen this error before, and would I be correct in thinking this is a problem with the specific user account?

Could you ask the user to describe how to reproduce the problem?

They said they are just authenticating as normal, nothing special. I will see what other info I can get from them (have they ever been able to auth successfully, did they choose to always allow access from our site, etc.)

Also helpful is the IDP they choose, and at which URL page did they see the error msg.
