Google approval video asking for things I can't provide

My demo video was rejected and the following things were asked for:

• Show how a user logs into your app and grants access to the requested scopes.
• Your app's OAuth Consent Screen must show your app's name, <My app name>.
• Enlarge the OAuth Consent Screen to show the URL including the Client ID (This is not required for native Android and iOS apps).
• Show how your app uses the requested scopes.
  
  

For the second point, my Oauth screen shows the domain, not the app name.  I don't know why, there seems nothing I can do about it.  For some reason, in my dev environment running on localhost, the app name shows.  But on the production server it shows the domain.

For the third point, I don't know what it's asking.  The Client ID is not shown on the OAuth screen.

How is this related to gcloud? What commands are you using to upload the video?
