[GCO4OSC] gc_dashboard MIA - not in manual install docs

I carefully followed the manual installation instructions (since I am
working with a modified OSCommerce site). And I do not see the Google
Checkout Dashboard on the admin screen.

The install docs do not mention the installation of the
gc_dashboard.php file - which appears to be where the dashboard code
'lives'.

Where is this module installed?

Hi Rick,

I did another install of GC last week & found I'd missed out copying
gc-dashboard so I guess it probably isn't in the instructions.

Copy the gc_dashboard.php file to [catalog]/admin/

You access it within the admin tool once the module is installed by
clicking on Google Checkout in the list of payment modules, then
there's a link not far from the remove & edit buttons, above the GC
version number.

That didn't work (copying the gc_dashboard.php file). There is no link
there on the GC/edit screen, see this code showing un-closed "<a ..."

I went through the manual instructions again, verified all was done
properly. There is a comment about the 'form' tag on step 6 being
incorrect, but I tried it both ways (a "form" and "/form") to no
avail.

So, GC is still broken.
