Cannot get past checkout confirmation

All seems to be working, and no errors anywhere (admin or store), but
i just keep getting bounced back to the checkout page. Any ideas what
it could be?

Does it actually seem to go to GC and then come back to osc, or just
refresh the page? What's the url?
