I am working on a prototype of a hospital training program for
hospital chaplains. It is designed to provide education on advance
directives, but more importantly, to help chaplains (and other health
care professionals) really listen to patients about what kinds of care
they would want. Thanks for this nice tool.

Look up the P.O.L.S.T. form as an example.

We are looking at using a POLST-type document. Texas has some very
unusual politics in terms of advance care planning that make it
difficult to incorporate some aspects of POLST. For the moment, we are
just starting to discuss prognosis and treatment earlier in the
process of dealing with chronic illness.
