In step 2, did you originally get a prompt to choose a barcode scanner to
use? If so, did you choose to use it by default (i.e., without prompting
again)? If so, perhaps that barcode scanner app is faulty. Unfortunately,
your Android now remembered that you'd like to use that app whenever an app
(not just Google Authenticator) needs a barcode scanner.
You can revert your "use by default" choice by going into System settings
-> Apps -> <the faulty barcode scanner app> -> Clear defaults. Once that's
done, scanning a QR code from Google Authenticator should start prompting
you to choose a scanner again.
Two more alternatives (but the above is preferred):
* Uninstall the faulty barcode scanner, OR
* Fire up a working barcode scanner, scan the account setup QR code for
Google Authenticator, and then let that barcode scanner launch Google
Authenticator instead.
