What steps will reproduce the problem?
1. Start authenticator
2. Click "Set up Account"
3. Click "Scan a barcode"

What is the expected output? What do you see instead?
I expect the camera to open up to scan a QR code. Instead, nothing happens
for a few seconds, then a black screen, then "Unfortunately, Authenticator
has stopped".

What version of the product are you using? On what operating system?
2.49. Using on Samsung Galaxy s4 running Android 4.3.

Please provide any additional information below.
I broke my phone last week and sent it in for warranty replacement. I
activated an iphone and installed authenticator on the iphone. Everything
was working perfectly. Got my replacement galaxy s4. I reinstalled all of
my apps and settings with titanium backup. I deleted all of the accounts
from authenticator expecting them to not work. I tried to reinstall with
barcode and I get the errors. Using typed codes work.

I have this issue and it seems AWS Virtual MFA app is not available for S4,
so I am really stuck.

I didn't manage to get it working with typed codes. Can anyone give me a
hint which code I need to use? I tried User Name and Access Key Id, which I
got from AWS, but this did not work.

I have a nexus 5 with android 4.4.3.
This is my first time to attempt using Google Authenticator but it crashes
every time I try to scan a barcode.

What steps will reproduce the problem?

I had the same issue on my new Nexus 5, but took a guess and installed the
ZXing Barcode Scanner app, in case it was a dependency. Google
Authenticator now works for scanning QR barcodes! A pretty ungraceful
handling of a crash due to a silent dependency!

Have a brand new Moto G 2014 edition, and have same problem. That's until I
read the comment #9 from jspar...
Installing ZXing Barcode Scanner fixed the issue for me too.
Google, please add the dependencies necessary to run the Authenticator.

I just started having this problem too. It was actually good exercise. I
backed up all my QR codes and Keys (which I hadn't done before). I
uninstalled and re-installed Authenticater. No deal. Still crashed when I
chose the scan barcode option.
I uninstalled all bar code readers on the phone and re-tried. Still
crashed. Then I installed ZXing from the play store and it started working
again.

Check you security setting for camera and make sure authenticator has
access enabled for it.

Good point. Still, crashing is not the way to go. It should alert the user
that they should grant the app access to the camera

I too found with my new iPhone 6 iOS8, the authenticator scan barcode kept
crashing, fixed when I checked security for authenticator (not camera &
photos security which I tried first!) and clicked the camera accessibility
there. Thanks for the heads up miten, I would never have thought to check
there if you hadn't suggested it!

Seems to me that the camera issue is related to iOS devices only. For
android, try the solution of jspar (#9). Install ZXing barcode scanner
(free), that workef for me.

**Best Solution** Install an app called DUO. It dose the same thing as
Google Authenticator, and it looks better! I've had the same issues with
Google Authenticator crashing as well. The DUO app is the best solution.

I had this issue but it would say UP has crashed. I uninstalled UP (the
Jawbone application). After that I would get an error message saying ZXing
barcode scanner needs to be installed but at least GA wouldn't crash.
Installing ZXing fixed it. No clue why having Jawbone UP installed causes
a crash. Odd

I had this issue on iOS8. Enabling the camera in the settings solved it.
The setting is under:

Settings -> Authenticator -> Privacy -> Camera

I have had a slightly different experience: it worked for Github and Google
(presumably in that order; that's the order they appear on my screen),
probably in the same session. Coming back days later to add a third site it
crashes at that same point [after 'scan a barcode', before camera loads]
every time.

Triga, I have the same phone, and had the same issue. have you tried to
install "ZXing Barcode Scanner" to solve the issue?. That worked for me and
has worked for others too.
1. Go to play store
2. Look for the free app "ZXing Barcode Scanner"
3. Once installed, the google authenticator is fixed

I experienced this issue on my Moto X 2014; as others have said simply
installing the "ZXing Barcode Scanner" app from the Play Store will resolve
the issue. However the Google Authenticator needs to be a much better job
at either gracefully failing or removing the dependency entirely.

PROBLEM SOLVED:

Google Authenticator has an app dependency.

Install ZXing Barcode Scanner and it will work again (or for the first time
depending on your case)

Same thing here with Android 4.4.4 on Moto X 2014 and fixed installing
ZXing barcode scanner.

It seems ZXing scanner dependency message box is only shown on pure android
devices. Other devices just causes a fatal error.

Fixed it!

I needs to have a QR code scanner installed.

I install "Google Goggles" works 100% now.
