Thank you for accepting me into the group, so I need help.
My goal is to automatically fill out any Google form with first and last name, phone number, city, email, full name, and add a photo and ID.

Hello!  We need some more information.  Do you want to prefill a form with data from a spreadsheet? 

What is the ultimate goal?

I have a spreadsheet

That has 2 column

One is STATE and the other column is Choice of HOSPITAL.

if I select LAGOS state  in the Google form I want all corresponingchoice of  hospital  that is in Lagos State in the spreadsheet, in the same vein for all other state
