Dependent dropdown in google forms

Is there any script that helps me in getting the dependent drop down list  in google forms?

As far as I know, you cannot do this with Google Forms.  However, you can achieve this using a Web App.

It uses "outdated" JQuery as well as a Bootstrap framework.  I am sure you can create the same thing with out these.
