File name has concatenate of OWNER

hi,
I have created a form that uploads files to Drive.

Than it creates a folder based on one of the fields in the form and moves the uploaded files into it.

However I've noticed that the file name has concatenate of " - <account-owner-name>" 

I'm using DriveApp.getFileById(fileId)

Did anyone else had such issue?

if YES how did you solved it?
