Hmm, that url has baytech.co.jp in it. 

Looks rather suspicious, and possibly a phishing attempt. 

Having said that, it appears to have the right formkey, same as a message here:

At a first glance, it might look like that. However, I believe that this is just because Abe-san used his baytech.co.jp Google Apps account when accessing the form. I know Baytech is a Japanese company which has been a legitimate Google Apps Reseller for a long time. Thus, I don't think it's a phishing attempt.
