I am trying to work out on the Admob since few days and I am also having the problem.
To make it simple to compare i edited the SampleBannerApp. I added two Ad banners, one banner with Sample AdUnitId and one banner with myApp AdUnitId.
Then tried the App in two ways i.e. using the Sample AppId and MyApp AppId inside the AndroidManifest.xml.
1) using my AppId : Both Test ads are showing properly in the Emulator but in real device only sample AdUnitId is showing.
2) using Sample AppId : again same result - Both Test ads are showing properly in the Emulator but in real device only sample AdUnitId is showing.
Please see the below screentshots with information in the image also.

As an aside, please note that adding the Banner AdView can be done either via XML layout or via programmatically. This shouldn't cause any issue; however, to further confirm, could you also share your sample implementation privately?

Thank you for sharing with me the requested details in private.
As per checking, I can confirm that there is nothing wrong with your banner implementation. In addition, I also tested your Ad Unit ID and I was able to load both test and live ads (see attached screenshot). Inconsistently, I also encountered the no fill error which indicates an ad-serving issue due to lack of ad in the ad inventory. Please note that this behavior isn't controlled by the Mobile Ads SDK.
Having that said, for ad-serving related concerns, you may use this Ads Troubleshooter or directly reach out to our Product Support Team as they're the appropriate team that could further assist you with such issues.

I have tested your Ad Unit IDs and I also encountered the no fill error. Please note that this error indicates an ad-serving issue and isn't controlled by the Mobile Ads SDK. You may want to use this Ads Troubleshooter or directly reach out to our Product Support Team so the product specialist there can further assist you with your ad-serving concerns.
With regard to the warning log (App does not have the required permissions to get location) you're getting, allow me to provide insights to this. Please note that our SDK automatically passes location data (if your app already requests location permissions from users) to serve location-based ads to users that are usually more relevant to their needs. You may refer to this article for more details about this.

Thank you for posting your concern. However, I'm afraid that we can only assist you with regard to the code implementation of the Mobile Ads SDK.
Since your concern is already account-related, I would suggest reaching out to our Product Support Team so they could further help you with your account issues.
MY REMARK: I Tried the app with and without the above codes but no success. (i put my 16 digit mobile device id in place of "my_device_id")
MY REMARK :since at present my app is to be installed directly and not through play store so i think this is not applicable please. suggest if this is correct.
Also i couldn't find this option, so in case when i plan to put my app on playstore then where will i see this option?
If you have changed your Keystore then you can fix this error just by replacing the Ad Unit.Still if you find this error then you don't worry and upload the App in Play Store or App Store andConnect your App With Admob. Hope this was Helpful for you.
MY REMARK: Again I think the above is applicable for playstore. so not applicable for my present app which is a direct install.
Also please confirm that when later i put up the app through playstore is there any relation in this?
You can install a mediation such as InMobi:In this way, if for whatever reason admob is not showing you ads,you can still show them from other ad networks.MY REMARK: Is this recommended?

I understand your concern. I'm sorry to hear that you're having troubles in reaching out our Product Support Team; however, they're the appropriate team that could help you with your ad-serving concerns. I hope you get a response from them the soonest time possible.
As an aside, allow me to provide insights on the suggestions mentioned below:
1) With this suggestion, it is always a best practice to use Test Ads when testing your app. However, when using the Enable test device feature, although this produces a production-looking ads, it isn't a guarantee that you will always have an ad if you're encountering an ad-serving issue.
2) I'm afraid that I can't comment to this any further as this is already out of our scope. You may want to reach out to the Play Store Support Team for this.
3) I'm afraid that I can't comment to this any further as this is also out of our scope. Fill rate concerns are best handled by our Product Support Team.

5) Mediation is a good feature in AdMob that lets you serve ads from multiple sources. You may refer to this guide for more details and to what specific mediation network you would want to integrate.

7) As for the demo period, I'm afraid that I can't also comment to this. Please note that your AdMob implementation is not the issue since you were able to load test ads. Your app is experiencing an ad-serving issue which isn't controlled by the Mobile Ads SDK. In addition, loading of ads on unpublished app really affects its fill rate.
