Can you confirm if you're using an Android app? If yes, the ad failed to 3 or ERROR_CODE_NO_FILL, means that the ad request was successful, but no ad was returned due to lack of ad inventory. Then, for an ad failing to load 0 or ERROR_CODE_INTERNAL_ERROR, means that something happened internally; for instance, an invalid response was received from the ad server.
To fix the error code 3, you may refer to common reason for ads not showing for additional scenarios, as well as the fixes for those. For error code 0, newly created Ad Units can take upto 2 hours to become active (SpinUp Time). Any Ad requests from newly created Ad Units would also receive Error Code 0 in response to Ad requests until they are ready to serve live ads. I would recommend testing our ad unit id.
