Haven't received my Address verification PIN on my postal address

Hi,

I had issued a request for the pin twice over a span of 3 months since June 2014, but have not received any thing in the mail box and hnec am unable to verify my address and monetize my apps.

Kindly inform me if I have missed anything out.
